I reinstall my OS but I forget to export my certificate from browser . How should I do now ? Should I request the certifiacte again or re-write the application form ?

Answer :

Yes, you should request a new user certificate but you don't need to send the application form to us again. You have already confirm the identity with ASGCCA manager before. So you just go through step 2 of request procedure to send CSR file to us. We will issue a new certificate to you and revoke an old one, your old certificate will not effective anymore.
